29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Crash Champions Collision Repair 2620 6th Avenue Garden City, CO 80631; Detail shop. On or about November 08, 2023, and at times prior, the employees were not provided suitable facilities for quick drenching or flushing of the eyes in the detail shop, while employees conducted forklift battery watering. This condition exposed employees to the risk of permanent damage to the eyes. b) 2620 6th Avenue, Garden City, CO 80631 - Detail Shop: On or around November 8 , 2023 and at time prior, employees in the detail shop were exposed to safety hazards in that a corrosive liquid (labeled UN 3266) was used as a degreasing agent without employees having access to an eyewash station.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) Crash Champions Collision Repair 2620 6th Avenue Garden City, CO 80631 Paint booth: On or about November 08, 2023, and at times prior, employees were exposed to health hazards while wearing a mandatory half-face respirator with facial hair that interfered with the seal. Employees wear respirators with long facial hair while spraying automobile parts in a spray finishing booth.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical a) Crash Champions Collision Repair 2620 6th Avenue Garden City, CO 80631 Storage room: On or before November 08, 2023, and at times prior, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. Employees worked in the storage room, where automobile parts are stored. The containers with unknown pink and green liquids did not have identifying information on them. This condition exposed employees to chemical hazards to the eyes, hands, and skin.
