29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   a) At or near 51 Queen Anne Rd, Bogota NJ -  Scaffold Platforms: Employees were exposed to Fall Hazards varying from 15 feet to 45 feet, as the lifelines being utilized to protect employees working from a pumpjack scaffold platform  were connected to Werner A210400 Anchor devices that had only 2-4 screws  on or about 11/13/23.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) At or near 51 Queen Anne Rd, Bogota NJ -  Balcony - 3rd level: Employee was exposed to fall hazards of approximately 25 feet while engaged stucco activities from the third floor balcony without proper means of fall protection, on or about 11/16/23.
