29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a) On or about November 20, 2023 and at times prior to,  at 7577 Atlantic Avenue, Delray Beach, FL 33446; employees engaged in insulation installation on top of the roof were exposed to falls through roof (hatch) holes from a 20 ft. elevation onto the concrete floor, and the employer did not ensure that employees were protected by fall arrest systems, covers, or guardrail systems erected around the roof (hatch) holes.

29 CFR 1926.503(a)(3):The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:   a) On or about November 20, 2023 and at times prior to, at 7577 Atlantic Avenue, Delray Beach, FL 33446; employees engaged in insulation installation on top of the roof were exposed to fall hazards through an unprotected roof hatch hole from a 20 ft. elevation onto the concrete floor, and the employer did not ensure that  workers were effectively trained in the recognition of the fall hazards and the procedures to control/minimize these hazards.
