29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: a) Front rooftop on the 1st Floor of the building. On or about November 28th, 2023, an employee was leaning over the parapet wall of the roof without fall protection and was exposed to falling approximately 12 feet from the 1st-floor rooftop to the ground level below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Location: a) Front rooftop on the 1st-Fl of the building On or about 11/28/2023, employees used a 32-foot ladder to access the front rooftop. The ladder used did not extend 3 feet above the intended landing surface.
