29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: Location: a) Left inside staircase on the 1st Floor of the building. On or about December 6th, 2023, an employee was priming and sanding the wall in the staircase. The scaffold platforms that an employee was on while priming and sanding the wall were not at least 18 inches wide.

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units. Location: a) Left inside staircase on the 1st Floor of the building. On or about December 6th, 2023, an employee was priming and sanding the wall in the staircase. Plastic buckets and cylinder blocks on the tread of the staircase were used to support the scaffold platform. Unstable objects were used to support the scaffold and its working platform.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person. Location: a) Left inside staircase on the 1st Floor of the building. On or about December 6th, 2023, employees erected a 1-frame high scaffolding system with plastic buckets and cylinder blocks on the tread of the staircase, which was used to prime and sand walls. The scaffold was not erected by experienced and trained employees under the supervision and direction of a competent person.
