29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   a. On or about December 12, 2023, at O2B  Apopka, employees were exposed to struck-by hazards, in that the use of head protection was not enforced by the employer while employees were engaged in framing activities above.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. On or about December 12, 2023, at O2B  Apopka,  employees were exposed to 12-feet 8-inch fall hazards while conducting framing work activities, in that, fall protection was not used.   b. On or about December 12, 2023, at O2B  Apopka,  employees were exposed to  13-feet 7-inches fall hazards while conducting framing work activities, in that, fall protection was not used.

On or about December 12, 2023, at O2B  Apopka, the employer did not have fall protection training certificates for employees exposed to fall hazards while conducting framing  activities.
