29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  A).	On or about 12/12/2023, at the site of 75-32 185th Street. Fresh Meadows, NY  Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and materials from other employees who on the scaffolds. Protective helmets were not worn.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   A.           On or about 12/12/23, at the site of 75-32 185th Street. Fresh Meadows, NY    Employees working on the fabricated frame scaffolds were exposed to the hazard of falling approximately 13 ft. to the ground. The working platforms were installed with only two scaffold planks.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs g(1)(i) through g(1)(vi) of this section, each employee working on a scaffold more than 10 above a lower level was not protected from falling to that lower level by the use of guardrails or personal fall arrest systems:  A.           On or about 12/12/23, scaffolds at 75-32 185th Street. Fresh Meadows, NY    Employees working from an unprotected tubular welded frame scaffold were exposed to a fall hazard of approximately 13 ft. while installing plywood to face of 2-story house.
