29 CFR  1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.  Location - 5 Hill Street, Woburn, MA 01701.  On or about 12/12/2023, an employee was standing on an eight foot step ladder which employee had set up in a closed position.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.  Location - Hill Street, Woburn, MA.  On or about 12/8/2023, an employee fell to lower level while working on floor decking. The employee was wearing personal fall protection system, but did not have the lifeline set properly to prevent him from falling to the lower level.
