29 CFR 1910.110(e)(4)(iii):Permanent and removable fuel containers shall be securely mounted to prevent jarring loose, slipping, or rotating, and the fastenings shall be designed and constructed to withstand static loading in any direction equal to twice the weight of the tank and attachments when filled with fuel using a safety factor of not less than four based on the ultimate strength of the material to be used. Field welding, when necessary, shall be made only on saddle plates, lugs or brackets, originally attached to the container by the tank manufacturer. On or about December 13, 2023, employees were utilizing a powered industrial truck with the propane tank not secured.

29 CFR 1910.37(b)(6):Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted. On or about December 13, 2023 exit signs were not illuminated so that employees would be able to see where the exit's are located during an emergency.
