(a) On or about December 13, 2023, the employee was not provided personal protective equipment such as gloves while operating a Stihl hedge trimmer (Model # HS 46C) exposing the employee to hand injury.

29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: (a) On or about December 13, 2023, the employee operated a Stihl hedge trimmer (Model # HS 46C) that was not maintained in a serviceable condition. The throttle body lever was required to be engaged at all times to prevent the trimmer from stalling out. This exposed the employee to a caught-in hazard and amputation hazard.
