Perimeter of second and third floors: On or about December 15, 2023, the employer failed to ensure that a wire rope used as a top rail in a fall protection system on floors #2 and #3 was flagged approximately every 6 feet with high visibility material.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Company:  On or about December 15, 2023, and times prior to, the employer did not maintain an individual written certification record for each employee.
