29 CFR  1926.752(c)(2): A firm, properly graded, drained area, was not readily accessible to work with adequate space for the safe storage of materials, and the safe operation of the erectors equipment.  a)  On January 31, 2024, and at times prior thereto, steel erection work occurred in mud and areas of standing water.  Equipment including scissor lifts, aerial lifts, and rough terrain forklifts were used throughout the site.  NOTE: Certification of corrective action AND supporting documentation as evidence of correction are required for this item.

29 CFR  1926.758(c): Rigid frames did not have 50 percent of their bolts or the number of bolts specified by the manufacturer (whichever is greater) installed and tightened on both sides of the web adjacent to each flange before the hoisting equipment is released:  a) Frame line 5: On January 31, 2024, and at times prior thereto, ASTM A354 BD bolts and heavy hex nuts were not tightened, including but not limited to those connecting structural steel at splice #1.  On the south side of column 5E  none of the 12 bolts were fully tightened.   b) Frame line 4:  On January 31, 2024, and at times prior thereto, ASTM A354 BD bolts and heavy hex nuts were not tightened, including but not limited to those connecting structural steel at splice #1.  On the south side of column 4E none of the 12 bolts were tightened.   c) Frame line 3:  On January 31, 2024, and at times prior thereto, ASTM A354 BD bolts and heavy hex nuts were not tightened, including but not limited to those connecting structural steel at splice #1.  On the top of column 3E at least 8 of the 12 bolts were not tightened.   NOTE: Certification of corrective action AND supporting documentation as evidence of correction are required for this item.

29 CFR  1926.761(c)(2)(i):The employer did not ensure each connector had been provided proper training in the hazards associated with connecting.  a) On January 31, 2024, and at times prior thereto, employees were engaged in the erection of an engineered metal building to be used as an aircraft hangar with 300 ft long spans.  Workers engaged in connecting had not received training to include bolt tightening requirements and torquing methods including for direct tension indicator (DTI) washers.    b) On January 31, 2024, and at times prior thereto, employees were engaged in the erection of an engineered metal building to be used as an aircraft hangar with 300 ft long spans.  Workers including those engaged in connecting had not received training to include erection procedures to ensure for the structural stability of long-span rafters.  NOTE: Certification of corrective action AND supporting documentation as evidence of correction are required for this item.

29 CFR 1926.754(a): Structural stability was not maintained at all times during the erection process.  a)  On January 31, 2024, and at times prior thereto, employees were erecting a steel hangar with 300 ft spans without intermediate support.  From Monday January 29, 2024, through Wednesday January 31, 150 feet long ground-assembled bays were placed and joined at the middle peak.  Each day the solid web structural I-beam rafters were observed bending/curved lengthwise, columns were observed out-of-plumb, and numerous 1/4 wire rope cables broke.  No temporary guy wires or other bracing were used.  NOTE: Certification of corrective actions AND supporting documentation as evidence are required for this item.
