29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) Pipeline Video Inspection, LLC DBA AIMS Companies; 100 West Center Street Johnsonburg, PA 15845-C10 Conveyor/tunnel area- on or about 1/25/2024 and times prior thereto: The employer did not evaluate contract work areas for potential permit required confined space hazards prior to initiating work operations. Employees entered a work area with high levels of Hydrogen sulfide, leading to an exposure incident involving rescue and medical treatment of multiple employees. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Pipeline Video Inspection, LLC DBA AIMS Companies; 100 West Center Street Johnsonburg, PA 15845- on or about 1/25/2024 and times prior thereto, employees were performing servicing and maintenance activities in the C10 Area under equipment including, but not limited to, C10 conveyor system. Before performing servicing and maintenance tasks, the C10 conveyor was not shutdown, isolated, and verification of isolation was not completed. b.) Pipeline Video Inspection, LLC DBA AIMS Companies; 100 West Center Street Johnsonburg, PA 15845- on or about 1/25/2024, employees were performing servicing and maintenance activities in the C10 Area. Before performing servicing and maintenance tasks, the chip bin steam was not shutdown, isolated, and verification of isolation was not completed before entering the C10 area. Employees were exposed to Hydrogen Sulfide, resulting in loss of consciousness, rescue, and emergency medical treatment. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.
