29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:  (a)Wilson Trailer Company at 415 East Oriole Avenue, Lennox, SD 57039: On or about and at times prior to February 7, 2024, the employer did not ensure employees working on the rear frame of the Model DWH-750 Pacesetter White were protected from fall hazards. This condition exposed employees to fall hazards greater than four feet.  (b)Wilson Trailer Company at 415 East Oriole Avenue, Lennox, SD 57039: On or about and at times prior to February 7, 2024, the employer did not ensure employees working on the rear frame of the Model DWH-700 Pacesetter were protected from fall hazards. This condition exposed employees to fall hazards greater than four feet.

This condition exposed employees to amputation and laceration hazards.

This condition exposed employees to caught-in hazards.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  (a)Wilson Trailer Company at 415 East Oriole Avenue, Lennox, SD 57039: On or about and at times prior to February 7, 2024, the employer did not ensure employees were protected from shock hazards in that a threaded electrical cable connector was pulled from the junction box exposing inner conductors. The junction box was attached to the Pacific Press 200 Ton Model No. 200 OBL -12/36, located on the back side of the press with the cable powering the light curtain. This condition exposed employees to shock hazards.  (b)Wilson Trailer Company at 415 East Oriole Avenue, Lennox, SD 57039: On or about and at times prior to February 7, 2024, the employer did not ensure employees were protected from shock hazards involving a power cord that had electrical tape covering the inner conductors which powered the control pedestal for the 200-ton Pacific Hydraulic Press. This condition exposed employees to electrical shock hazards.

29 CFR 1910.303(g)(1): The employer did not ensure that space about electric equipment and or sufficient access and working space was provided to permit ready and safe operation and maintenance of such equipment:  (a)Wilson Trailer Company at 415 East Oriole Avenue, Lennox, SD 57039: On or about and at times prior to February 7, 2024, the employer did not ensure there was sufficient access and working space involving electric equipment in that, materials were being stored within 36 inches of an electrical panel located in the compressor room. This condition exposed employees to shock and fire hazards.
