29 CFR  1910.1200(e)(2)(ii): The Employer's hazard communication program did not include methods the employer will use to inform the other employer(s) of any precautionary measures that need to be taken to protect employees during the workplace's normal operations and in foreseeable emergencies:      a.) Domtar Paper Company, LLC, C10 Conveyor area, on or about January 25th, 2024, and times prior thereto: Not all contract employees were adequately informed of the hazards and required precautions necessary for safely entering and working in the C10 conveyor tunnel areas. Contract employees entered the C10 conveyor tunnel to conduct chip vacuum operation without precautions and were exposed to Hydrogen Sulfide, resulting in loss of consciousness, rescue, and emergency medical treatment.     ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area:   a.) Domtar Paper Company, LLC, C10 Conveyor area, on or about January 25th, 2024, and times prior thereto: Not all contract employees were provided with effective training on methods to detect hazardous substances in their contracted works areas. Contract employees entered the C10 conveyor tunnel to conduct chip vacuum operation without precautions and were exposed to Hydrogen Sulfide, resulting in loss of consciousness, rescue, and emergency medical treatment. Domtar Paper Company, LLC, C10 Conveyor area, on or about January 25th, 2024, and times prior thereto: Not all contract employees were provided with effective training on the measures, procedures and work practices required to safely work in their contracted works areas.
