29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the requirements of section(c)(4)(ii)(A)-(c)(4)(ii)(D) of this standard.  a)	On or about June 11, 2022, at the facility located at 1149 North Margaret Street, Markesan, Wisconsin; the hazardous energy control procedure for Wax Press 3 (Asset No.: WP-P003) did not account for the hazardous energy source of gravity.  b)	On or about June 11, 2022, at the facility located at 1149 North Margaret Street, Markesan, Wisconsin; the hazardous energy control procedure for Wax Press 4 (Asset No.: WP-P004) did not account for the hazardous energy source of gravity.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  a.	On or about February 15, 2024, at the facility located at 1149 North Margaret Street, Markesan, Wisconsin, the guarding on Wax Press #3 (Asset No.: WP-003) was inadequate to prevent employees from access during operation.  The guarding doors were closed by a barrel bolt and did not require special tools to open the guard.  b.	On or about February 15, 2024, at the facility located at 1149 North Margaret Street, Markesan, Wisconsin, the guarding on Wax Press #4 (Asset No.: WP-004) was inadequate to prevent employees from access during operation.  The guarding doors were closed by a barrel bolt and did not require special tools to open the guard.
