At a worksite located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to eye injury hazards, in that eye protection were not utilized by employees who were operating pneumatic nail guns to nail down the decking.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 4:12 pitch roof, at a worksite located at 8513 Howell Drive, Jacksonville, Florida 32208:  On or about February 15, 2024, the employer exposed five employees to fall hazards, in that, fall protection was not utilized by employees while nailing down decking and installing roof shingles.  The employees were exposed to falls hazards ranging from 8 feet to 16-feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. Front left side of home located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to fall hazards in that, the ladder used to gain access to the roof, the side rails did not extend three feet above the roof and was not tied off, exposing employees to an 8 feet fall hazard.  b. Left side of home located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to fall hazards in that, the ladder used to gain access to the roof, the side rails did not extend three feet above the roof and was not tied off, exposing employees to an 8 feet fall hazard.
