Location: Worksite; doorway leading from the lobby area into the larger communal section.  a) On or about 3/4/2024, the employer did not ensure that the passageway leading from the lobby area into the larger communal area, was kept in a clean and orderly condition. An electrical cord was partially suspended off the ground in the doorway between the two rooms, posing a tripping hazard to employees.

Location: Worksite; lobby area.  a) On or about 3/4/2024, an extension cord was series-connected to a relocatable power tap (RPT), which was used to energize a second RPT located in another room. The device was not used in accordance with the manufacture's listing and labeling specifications.

Location: Worksite; lunchroom/breakroom area.  a) On or about 3/4/2024, employees were exposed to an electrical shock from exposed wiring from the electrical panel missing the dead front cover.
