OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm in that employees were exposed to crushed by hazards due to the unsafe operation of automotive lifts:  Service Area - Auto lifts were operated unsafely and/or with defects, in the following instances:  a) Lift #1 (SN: KSM16E0010) - Lift was not inspected annually. Cables needed to be adjusted or replaced.  The lock bar required replacement.  Left side ramp did not lower/engage when the lift was raised. Three (3) sheave covers need to be installed.   b) Lift #2 (SN: NC521H0010) - Lift had not been inspected since installation (2021). The overhead safety cut off switch was not wired. Lift pads on adapters were missing. Plungers and stop were seized on drivers front arm. c) Lift #3 (SN:  NGK18C0003) - Lift had not been inspected since installation (2018).  The overhead safety cut off switch was not wired. Cables were not adjusted.  d) Lift #4 (SN: NGK18C0001) - Lift had not been inspected since installation (2018).  The overhead safety cut off switch was not wired.  Adapters were not equipped with pads.  e) Lifts #1, #2, #3, and #4 - Lift operators were not trained, and the training was not documented.  f) Lifts #1, #2, #3, and #4 - Documents, indicating the lifting locations of vehicles, were not available for lift operators.
