29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) New commercial construction located at 580 Peachtree Parkway, Cumming, GA 30040, Roof: On or about 03/11/2024, the employer exposed employees to fall hazards of approximately 21-feet, to the lower ground level, in that the ladder used to access the roof of the structure was not extended at least 3-feet or secured above the upper landing surface.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) New commercial construction located at 580 Peachtree Parkway, Cumming, GA 30040. On or about 03/11/2024, the employer failed to inspect all electrical extension cords used to provide power to electric tools employees were using, in that a orange and black 100-ft. electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
