Facility Wide - On or about March 13, 2024, the employer failed to certify that a workplace assessment had been performed. Employees were exposed to hazards that required the use of personal equipment including, but not limited to, eye and face protection.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a. Department 15, Tank 15D3 and 15D4 - On or about March 06, 2024, the employer did not ensure employees were provided and required to use appropriate eye and face protection when exposed to eye and face hazards during the transferring of hazardous liquid chemicals. Employees required to pump and pour Nitric Acid without the appropriate eye and face protection were, thereby exposed to eye and face injuries.
