The program shall enable each employee to recognize hazards related to ladders and stairways and shall train each employee in the procedures to be followed to minimize these hazards.  a) On or about and at times prior to March 27, 2024, at the previously addressed inspection site, employees accessed the roof of a residential building by using portable ladders as extension ladders with side rails that did not extend at least 3 feet above the upper landing surface or roof line, exposing the employees to falls.  A training program was not provided for the employees.
