29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Hydraulic Shop Floor: On or about April 3, 2024 and times prior, employees used a pedestal grinder to knock burrs off parts.  The tongue guard on the grinder was not adequately adjusted resulting in a 1 ? inch opening between the abrasive wheel and the top of the grinder.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 psi, and then only used with effective chip guarding and personal protective equipment.  a) Hydraulic shop; On or about April 3, 2024 and times prior, employees used compressed air to clean off equipment, tools and materials.  The air gun was not equipped with a safety nozzle and the air blew at above 80 psi.  b) Maintenance shop; On or about April 3, 2024 and times prior, employees used compressed air to clean off equipment, tools and materials.  The air gun was not equipped with a safety nozzle and the air blew at above 100 psi.
