29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about April 4, 2024, on lot #93 on the north side of the project site, employees exposed to fall hazards worked without adequate fall protection while performing roofing operations at heights greater than six feet above the ground.  b) On or about April 4, 2024, on lot #94 on the north side of the project site, employees exposed to fall hazards worked without adequate fall protection while performing roofing operations at heights greater than six feet above the ground.  c) On or about April 4, 2024, on lot #95 on the north side of the project site, employees exposed to fall hazards worked without adequate fall protection while performing roofing operations at heights greater than six feet above the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 4, 2024, on Lot# 93, 94, and 95, employees utilized multiple extension ladders to access the second level work area while the ladders were not extended three (3) feet above the landing:  a) Ladder #1: Werner Twenty (20) foot extension ladder, facing the west side of the structure on Lot# 93, used to access the upper landing surface.  b) Ladder #2: Werner Twenty (20) foot extension ladder, facing the north side of the structure on Lot# 93, used to access the upper landing surface.  c) Ladder #2: Werner Twenty (20) foot extension ladder, facing the north side of the structure on Lot# 94, used to access the upper landing surface.  d) Ladder #2: Werner Twenty (20) foot extension ladder, facing the east side of the structure on Lot# 95, used to access the upper landing surface.
