29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about April 8, 2024, at a jobsite located at W23N9484 Pebble Brook Way, Lisbon, WI 53089, an employee was operating a pneumatic nail gun without eye protection while installing decking on the roof of a new residential home. This exposed the employee to a struck by hazard and eye impalement injury.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  (a) On or about April 8, 2024, at a jobsite located at W23N9484 Pebble Brook Way, Lisbon, WI 53089, the employer did not ensure that an employee working from a JLG SKYTRAK rough terrain forklift (Model: 6042) at an approximate height of 10 feet 9 inches was protected from falling to the lower level.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about April 8, 2024, at a jobsite located at W23N9484 Pebble Brook Way, Lisbon, WI 53089, an employee was using a Werner extension ladder erected on the left front side of the house, to access the roof, which did not extend three (3) feet past the landing area, therefore exposing employees to a fall hazard greater than six (6) feet.
