29 CFR  1926.1053(b)(4): Ladders were not used for the purpose for which they were designed:  (a)  913 Highland Circle, Richmond Hill, GA 31324: On or about April 12, 2023, the employer exposed an employee to a fall hazard, in that employees used a stepladder in a partially closed position to access an upper level.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Roof: On or about April 12, 2024, the employer exposed employees to a fall hazard, in that fall protection was not used while employees performed residential roofing work with up to a 25-foot eaves to ground fall distance.  Roberto Lopez was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1634866, citation number 2, item 1 and was affirmed as a final order on February 1, 2023, with respect to a workplace located at 105 St. Phillip Way, Springfield, Georgia.
