29 CFR  1926.502(a)(2):Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection.  a.) On or about April 22, 2024, the employer failed to install a fall protection system for employees conducting roofing work. As a result, employees were exposed to a fall hazard of 14 feet 8 inches.

29 CFR  1926.502(f)(1)(i)Warning lines shall consist of ropes, wires, or chains, and supporting stanchions erected as follows:  When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge.  a.) On April 22, 2024, the employer did not ensure employees were protected from falling in that employees performing roofing work on a commercial flat roof with warning lines not erected at least 6 feet away from the roof edge were exposed to fall hazards of 14'8".

29 CFR  1926.1053(a)(24):The side rails of through or side-step fixed ladders shall extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder. For a parapet ladder, the access level shall be the roof if the parapet is cut to permit passage through the parapet; if the parapet is continuous, the access level shall be the top of the parapet.  a.) On or about April 22, 2024, the employer failed to ensure their ladder, being used to access the shopping plazas rooftop was extended the required 42" above a parapet wall as a result, employees were exposed to a fall hazard of 16'1".
