29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about May 1, 2024, an employee engaged in residential construction activities was exposed to fall hazards in excess of 6-feet without using any form of fall protection. This condition exposed the employee to fall and struck-by hazards.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section. On or about May 1, 2024, an employee was dry-cutting concrete roof tiles to roof a new residential home under construction without any respiratory protection or other engineering controls. This condition exposed the employee to hazards of respirable crystalline silica.
