On a residential roof, at a temporary worksite located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to nail down the decking.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a residential roof, at a temporary worksite located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to fall hazards ranging from 8 to16-foot, in that, fall protection was not utilized by employees while nailing down decking and installing roof shingles.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a.  At the front left elevation of a residence, at a temporary worksite located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to fall hazards ranging from 6 to 8-foot, in that the side rails of a portable multi-position ladder did not extend 3-feet above the upper landing surface of the roof as employees used the ladder to gain access to the roof.  b. At the left elevation of a residence, at a temporary worksite located at 8513 Howell Drive, Jacksonville, FL 32208: On or about February 15, 2024, the employer exposed employees to fall hazards ranging from 6 to 8-foot, in that the side rails of a portable multi-position ladder did not extend 3-feet above the upper landing surface of the roof as employees used the ladder to gain access to the roof.
