29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a. Interior of house and front side of roof: The Electro-Hydraulic Rebar Cutter used for cutting rebar was powered by two extension cords connected in series (daisy chained) and plugged into an outlet box that was located on the exterior/back of the house. The employee was exposed to electrical and fire hazards. b. Exterior/back of the house: The outlet boxes used to provide power to the job site were designed to be installed in a wall but were surface mounted on the exterior of the back of the house. Employees were exposed to electrical and fire hazards.
