Hoge Lumber Company on or about May 9, 2024 at the location of 701 South Main Street New Knoxville, Ohio 4587: The employer did not ensure established lockout procedures were followed in sequence to protect employees from potentially hazardous energy when performing service and maintenance task. Due to this condition, operators tasked with changing the Tannewitz Table Saw (Model XJ)  blade were exposed to hazards associated with being caught in rotating parts and nip points.

Due to this condition, Operators tasked with operating Tannewitz Table Saw (Model XJ)  were exposed to hazards associated with lacerations created by the rotating saw blade.

On April 10, 2024, a recordable injury occurred when an employee sustained an amputation of the tip of their left thumb; column F did not describe the object or substance that caused the injury (Case No. 1).
