29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.) Fostermation, Inc. 200 Valleyview Drive Meadville, PA 16335 Secondary Department, on or about May 02, 2024, and times prior thereto: Employees are exposed to hazards associated with rotating parts at the Swagger machines. The Swagger machines do not have guarding to prevent employees from contacting the rotating bolt heads.  b.) Fostermation, Inc. 200 Valleyview Drive Meadville, PA 16335 Secondary Department, on or about May 02, 2024, and times prior thereto: Employees are exposed to hazards associated with the stop adjustment rod at the Flat Dye machine #FD2. The stop adjustment rod is not guarded to prevent accidental contact to employees working at the Flat Dye machine.  c.) Fostermation, Inc. 200 Valleyview Drive Meadville, PA 16335 Secondary Department, on or about May 02, 2024, and times prior thereto: Employees are exposed to hazards associated with the stop adjustment rod at the Flat Dye machine #FD1. The stop adjustment rod is not guarded to prevent accidental contact to employees working at the Flat Dye machine.

Fostermation, Inc. 200 Valleyview Drive Meadville, PA 16335 Secondary Department, on or about May 08, 2024, and times prior thereto: Employees are exposed to hazards associated with unguarded flywheels at the Flat Dye machines. The knurling machine #FD2 had the flywheel cover removed while the machine was in operation. The flywheel was 2 feet above the floor and was not completely guarded.   b.) Fostermation, Inc. 200 Valleyview Drive Meadville, PA 16335 Secondary Department, on or about May 08, 2024, and times prior thereto: Employees are exposed to hazards associated with unguarded flywheels at the Flat Dye machines. The Waterbury Flat Dye Machine #FD1 had the flywheel cover removed while the machine was in operation. The flywheel was 2 feet above the floor and was not completely guarded.
