29 CFR 1910.106(e)(6): In locations where, flammable vapors may have been present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition such as but not limited to open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: On or about May 17, 2024, and at time prior thereto, at the washout bays and the shaker units, employees conducted dirty saltwater separations and washouts while diesel truck engines in the washout bays were allowed to continue running. This condition created an ignition source for flammable gasses.
