29 CFR  1910.307(b):Areas designated as hazardous (classified) locations under the Class and Zone system and areas designated under the Class and Division system established after August 13, 2007 were not properly documented:  On or about May 17, 2024, and at times prior thereto, the employer did not ensure that work zones were electrically classified and documented to effectively control the potential for explosions in areas with hazardous atmospheres at this worksite.

29 CFR  1910.307(c)(3):The equipment used in the hazardous(classified) location was not of the type and design that would provide protection from the hazards arising from the combustibility and flammability of vapors, liquids, gases, dusts, or other fibers involved:  On or about May 17, 2024, and at times prior thereto, the employer did not ensure that equipment was rated to be utilized in areas with hazardous atmospheres at this worksite.  The employer utilized a circular metal base fan was used by the operators (Mud Reception Tank) for comfort ventilation that was not rated for use in a flammable area.
