29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a) On or about May 23, 2024, in the automotive shop the employer did not ensure open exhaust ventilation holes in the floor measuring seven (7) inches in diameter were provided with a cover or guardrail. Employees were thereby exposed to trip and fall hazards.

Employees were exposed to electrical hazards from the pathway being less than thirty-six (36) inches wide.
