The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.   29 CFR 1926.602(d) NOTE: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.178(l) of this chapter.  a) On or about May 28, 2024, the employer did not certify that each forklift operator had been trained and evaluated.
