On or about May 31, 2024, at the above addressed jobsite, an employee using a hammer to remove plywood, was not wearing safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  b.  On or about May 31, 2024, at the above addressed jobsite, an employee using a portable circular saw, was not wearing safety glasses, thereby exposing the employee to the hazards associated with eye injuries

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about May 31, 2024, at the above addressed jobsite, an employee performing residential roofing operations, fifteen (15) feet, seven (7) inches above the ground, was not protected from falls, thereby exposing employee to fall hazards.  b.  On or about May 31, 2024, at the above addressed jobsite, employees performing residential roofing operations, twelve (12) feet four (4) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

On or about May 31, 2024, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.
