Jobsite: On or about June 6, 2024, the employer did not ensure that an employee clearing debris from a trench measuring approximately 6 feet, 5 inches deep was protected from cave-ins by using an adequate protective system, or by sloping the walls to prevent the exposure to a cave-in.

Jobsite: On or about June 6, 2024, the employer did not ensure that an employee clearing debris from a PVC bank of wiring in a trench measuring approximately 6 feet, 3 inches wide by 9 feet, 10 inches long and approximately 6 feet, 5 inches deep was trained to recognize and avoid the hazards of working in a trench.

Jobsite: On or about June 6, 2024, the employer did not ensure that an escape ladder was set up within 25 feet of an employee removing debris from a PVC bank in a trench measuring approximately 6 feet, 5 inches deep.
