29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Worksite, 185 9th street, Hicksville, NY, 11801- An employee was exposed to falling approximately 8ft while working from a fabricated frame scaffold. The working level had only two planks installed; on or about June 13, 2024.  b) Worksite, 185 9th street, Hicksville, NY, 11801- Two employees were exposed to falling approximately 14ft while working from a fabricated frame scaffold. The working level had only three planks installed; on or about June 13, 2024.  c) Worksite, 185 9th street, Hicksville, NY, 11801- An employee was exposed to falling approximately 20ft while working from a fabricated frame scaffold. The working level had only one plank installed; on or about June 13, 2024.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a) Worksite, 185 9th street, Hicksville, NY, 11801- Employees accessed a three-tier fabricated frame scaffold by climbing the frame. A ladder was not provided to access the working level of the scaffold approximately 14-feet above the ground; on or about June 13, 2024.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

The stucco contains portland cement and silica sand. The employer did not implement a written hazard communication program; on or about June 13, 2024.
