29 CFR  1926.20(b)(1):  The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Maynor Rodas on or about June 13, 2024, at the address of 206 E. Bridge Northwood OH 43619 (Building 55). The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Bridge Northwood, OH 43619 (Building 55): The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.

Bridge Northwood OH 43619 (Building 55): The employer did not ensure that employees were protected from head injury hazards by protective hardhats while working on the ground below as employees above actively installed prefabricated wooden trusses. Employees operating in this condition were exposed to struck by injuries from falling/flying objects such as tools and building materials.

Bridge Northwood OH 43619 (Building 55): The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns/electric circular saw during framing work activities on a residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

Bridge Northwood OH 43619 (Building 55): The employer did not ensure employees were protected from falls while performing  framing activities atop a two-story multi-family residential structure. Employees installing prefabricated wooden roof trusses from the exterior top plate were exposed to fall hazards of approximately 11 feet to the ground below.
