29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Hy-tech Mushroom Compost Inc. located at 155 Valley Rd. West Grove, PA  19390 -  On or about June 11, 2024, the employer failed to establish an energy control program for employees removing debris jammed inside the supplement belt. An employee removed a stone jammed in the supplement belt without isolating the machine from the energy source, exposing the employee to broken bones and amputation hazards.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in servicing and/or maintenance of equipment:  a) Hy-tech Mushroom Compost Inc. located at 155 Valley Rd. West Grove, PA  19390 -  On or about June 11, 2024, the employer failed to ensure hazardous energy control procedures were developed and utilized for a supplement belt machine. Employees were required to clear a jam from the conveyor portion of the supplemental belt machine, exposing employees to broken bones and amputation hazards.
