29 CFR  1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:   a) Jobsite located at 155 Valley Road, West Grove, PA: The employer did not ensure that the energy isolating device, such as a key for a Parnell soil turning machine, was physically located in manner to isolate the energy source before clearing a jam, thereby exposing employees to hazards associated with the release of stored energy. 29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Jobsite located at 155 Valley Road, West Grove, PA. - The employer failed to document and utilize the hazardous energy control procedures that were developed for the soil turning machines. Employees were required to clear a jam from the soil turning machine, exposing employees to struck-by and caught-in-between hazards.  Condition noted on or about May 1, 2025.  Hy-tech Mushroom Compost Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1755357, citation number 1, item number 1/b and was affirmed as a final order on 2/7/2025, with respect to a workplace located at 155 Valley Road West Grove, PA.
