29 CFR  1915.115(a)(1):When used to transfer materials or equipment from or to a vessel, cranes which are placed onboard barges, other vessels, or on wingwalls of floating drydocks, were not tested and certified in accordance with the standards provided in 29 CFR 1919 of Hoisting and Hauling Equipment by accredited persons.  (a) Columbia River South of Hayden Island, adjacent to Diversified Marine, Inc., 1801 N. Marine Drive, Portland, Or. 97217, Vessel DB Freedom, on or about June 21, 2024 and times prior thereto: Employer failed to test and certify crane barges in accordance with the standards provided in 29 CFR 1919 of hoisting and hauling equipment by accredited persons. Exposing employees to potential serious struck by, crushed by, pinched, and pinned by occupational health and safety hazard injuries. 29 CFR  1915.55(e)(1):Fuel gas and oxygen manifolds did not bear the name of the substance they contain in letters at least one (1) inch high which shall be either painted on the manifold or on a sign permanently attached to it.  (a) New build dry dock, deck, Vessel Rowdy, on or about June 21, 2024 and times prior thereto: Employer failed to identify fuel gas and oxygen manifolds with the name of the substance they contained in letters at least one (1) inch high, exposing employees to industrial hazards resulting in potential serious injuries and disability.  (b) Diversified Marine, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1915.55 (e) (1), which was contained in OSHA inspection number 1632873, citation number 3, item number 1 and was affirmed as a final order on 01-31-2023, with respect to a workplace located at 1801 N. Marine Drive, Portland , Oregon 97217.

29 CFR  1910.303(b)(2):Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  (a) Outside break area beside maintenance shop area, on or about June 21, 2024 and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in potential minor burns and electrical shock.   For Reference:  Underwriters Laboratories (UL), refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.

(a) Main Office, hallway , on or about June 21, 2024, and at times prior thereto: Employer failed to inspect, maintain, and test portable dry chemical fire extinguishers according to NFPA 10-2002 Standards for Portable Fire Extinguishers exposing employees to potential burns, injuries, and malfunctions.  (b) Extinguisher #12 maintenance shop, on or about June 21, 2024, and at times prior thereto: Employer failed to inspect, maintain, and test portable dry chemical fire extinguishers according to NFPA 10-2002 Standards for Portable Fire Extinguishers exposing employees to potential burns, injuries, and malfunctions.  (c) Extinguisher #43, maintenance shop area, on or about June 21, 2024, and at times prior thereto: Employer failed to inspect, maintain, and test portable dry chemical fire extinguishers according to NFPA 10-2002 Standards for Portable Fire Extinguishers exposing employees to potential burns, injuries, and malfunctions.
