On the side of a residential structure, located at 12684 European Beach Road, Jacksonville, FL, on or about June 26, 2024: The employer exposed employees to fall hazards, in that the 25-foot Werner Multi positional ladder used to access the top of the structure did not extend three feet above the eave.

Meraki Solar, LLC.  located at 21 N New Warrington Road, Pensacola, FL: On or about June 26, 2024, and times prior thereto, the employer failed for calendar year 2023, to electronically submit information from the OSHA Form 300A or equivalent by 03/02/2023. The establishment employed approximately 167 employees and was classified in the North American Industrial Classification System as NAICS-238210 during calendar year 2023.
