On or about June 26, 2024, at the jobsite, employees engaged in roofing installation work in a two-story residential house roof at heights greater than 6 feet did not have fall protection and were exposed to fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about June 26, 2024, on the north side and south side of the house, employees were accessing a two-story roof with ladders that did not extend 3 feet above the roof line.
