29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  (a) 1900 E. 3rd St, DURANGO, CO 81301: On or about June 26, 2024, and at times prior, the employer did not certify that hazard assessments had been performed prior to employees conducting tree trimming tasks over a waterway. Employees operating chainsaws over a waterway without personal protective equipment were exposed to laceration and drowning hazards.
