29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) a) On or about July 8, 2024, in a trench between 18 and 24 feet deep, located at 1003 Otter Creek Loop in Hazen, ND. Subcontracted employees were replacing a sanitary sewer. Employees were exposed to the hazards of no means of egress, trench boxes installed without tabulated data, and lack of protective systems.
