29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Worksite, 2 River Chase Drive, Rensselaer, NY, 2nd Level of Roof - On or about July 9, 2024, employees were accessing the roof of a residential building from a portable extension ladder approximately 25-feet above the ground. The ladder side rails extended approximately 17-inches above the landing surface. b) Worksite, 2 River Chase Drive, Rensselaer, NY, Front Roof - On or about July 9, 2024, employees were accessing the roof of a residential building from a portable extension ladder approximately 10-feet above the ground. The ladder side rails extended approximately 24-inches above the landing surface. c) Worksite, 2 River Chase Drive, Rensselaer, NY, 1st Level Porch - On or about July 9, 2024, employees were accessing the roof of a residential building from a portable extension ladder approximately 10-feet above the ground. The ladder side rails were approximately 6-inches below the landing surface of the roof, and the employer did not ensure the ladder was secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist the employee in mounting and dismounting the ladder. d) Worksite, 2 River Chase Drive, Rensselaer, NY, Side Roof by E21 - On or about July 9, 2024, employees were accessing the roof of a residential building from a portable extension ladder approximately 10-feet above the ground. 29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: a) Backside Roof - On or about July 8, 2024, an employee was exposed to fall hazards up to approximately 20 feet when using an extension ladder to access the roof with the ladder deflecting and not secured that caused the ladder to slip off its support. The employer did not have a ladder safety training program for the employee. b) 2nd Level of Roof - On or about July 9, 2024, employees were exposed to fall hazards of approximately 25 feet due to using an extension ladder to access a roof with the extension ladder extended approximately 1.5 feet above the upper landing surface of the roof. The employer did not have a ladder safety training program for the employees. c) Corner of Front Roof - On or about July 9, 2024, an employee was exposed to fall hazards of approximately 10 feet due to using an extension ladder to access a roof with the extension ladder extended approximately 2 feet above the upper landing surface of the roof. The employer did not have a ladder safety training program for the employee. d) 1st Level Porch - On or about July 9, 2024, an employee was exposed to fall hazards of approximately 10 feet due to using an extension ladder to access a roof with the extension ladder approximately 6 inches below the upper landing surface of the roof, and the employer did not ensure the ladder was secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist the employee in mounting and dismounting the ladder.
