29 CFR 1910.178(m): Truck operations: (5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline.    a) On or about July 22, 2024, the employer failed to ensure that when a forklift was left unattended, its load-engaging means (forks) were fully lowered in that the employer permitted a forklift to be left unattended with the forks raised, supporting the bridge girder from a tipped overhead gantry crane as a secondary support, exposing employees to struck-by hazards.

29 CFR 1910.179(n) Handling the load: (3) Moving the load: (i): The load shall be well secured and properly balanced in the sling or lifting device before it is lifted more than a few inches.  a) On or about July 22, 2024, the employer failed to ensure that a 38 metric-ton manufactured module was properly secured before lifting it with a 60-ton ENERP AC gantry crane. Prior to conducting a secondary lift of the module to center the module on a trailer, a shackle was inadvertently removed and not properly communicated to the crane operator. This resulted m the load shifting during the initial lifting process resulting m the gantry crane tipping forward.

29 CFR 1910.179(n): Handling the load: (3): Moving the load: (x): The employer shall insure that the operator does not leave his position at the controls while the load is suspended.    a) On or about July 22, 2024, the employer failed to ensure that the crane operator did not leave his position at the controls when an overhead gantry crane was being used to hold a suspended load. The employer permitted a tipped gantry crane suspended by an existing overhead gantry crane overnight, exposing employees to struck-by hazards.
