29 CFR  1910.176(a):Aisle(s) or passageway(s) were not kept in good repair:  a) Evergreen Recycling LLC on or about July 23, 2024, at the address of 202 Water Tower Dr. Clyde, OH; The employer did not ensure that the pathways in the "Bale Lot" where class IV powered Industrial trucks were utilized was maintained in a way to provide level operating surfaces for safe transport, and storage of baled product. Employees operating powered industrial trucks in this location during the handling and placement of compressed plastic bales were exposed to struck by injuries from displaced loads during travel and staging.

29 CFR  1910.178(o)(1):The employer did not ensure that only stable or safely arranged loads were handled:  a) Evergreen Recycling LLC on or about July 23, 2024, at the address of 202 Water Tower Dr. Clyde, OH: The employer did not ensure that bales of compressed plastic bottles were supported and arranged in a way to provide stable and safe transport. Operating employees as well as employee working in proximity of the powered industrial truck traveling with loads in this condition trucks were exposed to struck by hazards.

Employees working near the bales of compressed plastic bottles in this condition, were exposed to struck by injuries.
